- Home Visit with NDIS Participant and Occupational Therapist/OTA to determine needs/wants.
- Referral to builder of choice/NDIS registered builder
- Completion of ‘existing’ and ‘proposed’ drawings
- Home Visit Consultation with OT/OTA, NDIS participant and builder to confirm plans
- Completion of NDIS Complex Home Modifications (CHM) paperwork
- Submission of CHM paperwork with quotation to the NDIS for approval
